The National Network of Public Health Institutes, the Centers for Disease Control and Prevention, the Public Health Institute (CA) and Community Commons collaborated on a systematic review of the following:

1. How organizations defined a “healthy community”;
2. How local policy databases supported healthy communities; and
3. How nonprofit hospitals, health departments, and other organizations were conducting Community Health Needs Assessment and Community Health Improvement (CHI) processes.

This body of work recognizes the need to harmonize steps, tools available and action across community stakeholders. The resulting products are housed on Community Commons, CHI Hub and CHI Journey and include a suite of online tools and technical assistance resources that promote CHI planning, implementation and continuous quality improvement.
