The State Innovation Model (SIM) Initiative

Analyzing states’ health innovation plans to determine the extent of collaboration between public health and health care.

Our Work

This analysis assessed the degree to which State Health Innovation Plans (SHIP) promote the integration of public health and healthcare (PH-HC) and bridge gaps between the public health and healthcare fields. In close collaboration with CDC’s Office for State, Tribal, Local and Territorial Support (OSTLTS), NNPHI worked with the Louisiana Public Health Institute to conduct a six-week systematic qualitative analysis of 18 State Health Innovation Plans.

Our Impact

Our analysis exposes continued gaps in PH-HC integration and contributes to a growing body of evidence supporting health care. In addition, our assessment will inform preparations to meet states’ technical assistance need while shining a light on a critical shared goal: to promote the health and well-being of all people by enabling sustained collaboration between the public health and healthcare systems.

More about the State Innovation Model

Established by the Affordable Care Act, the Centers for Medicare and Medicaid Services Innovation Center (CMMI) developed the State Innovation Model initiative to support states in testing strategies that will achieve the “Triple Aim”: enhance the healthcare delivery system, reduce costs, and improve population health.
