Health & Housing: Evidence-Informed Policy & Practice Approaches for Healthy Aging

About the Health & Housing Project

In partnership with CDC’s Office of the Associate Director for Policy and Strategy (OADPS) and the Department of Housing and Urban Development (HUD), NNPHI and the Georgia Health Policy Center (GHPC) will identify evidence informed approaches for affordable housing programs that coordinate health, wellness, and supportive services to help older adults remain healthy, age in their community, and reduce use of costly health care services. This project will accelerate translation of evidence-informed policies and practices supported by CDC subject matter experts for older adults in HUD-assisted housing.

Our Work

Through this work, project partners will conduct an environmental scan of peer reviewed, grey literature and emerging practices related to available practice and finance models for supportive services/care coordination with a focus on serving older, low income adults residing in federally assisted housing; engage in critical conversations with CDC subject matter experts to identify additional resources, and host a series of virtual federal interagency stakeholder meetings on Aging in Place with HUD, CDC and other HHS agencies.

Our Impact

The overarching goal for this project is to identify evidence-informed population health interventions that coordinate health, wellness, and supportive services to help older adults remain healthy, age in their communities, and reduce their use of health care services. Going forward we hope to build a sustainable partnership with CDC and HUD to advance shared priorities related to health and housing.
