Community Health Assessments and Community Improvement Plans (CHA/CHIP) can be intimidating but also rewarding projects. Learn how a local health department has implemented their CHA/CHIP using the Mobilizing for Action through Planning and Partnerships (MAPP) process and recommendations from the Public Health Accreditation Board (PHAB). Our presenter will guide us through the time and resources that it takes to conduct these processes “by the book”.
Valuable lessons learned will be discussed as Ms. Manzella shares the pitfalls and successes experienced in data collection methods, conducting assessments, understanding what works in the community, and using a continuous improvement model.
We will conclude with strategies for building and sustaining partnerships that transcend MAPP cycles and creating CHIP linkages to other community plans and structures.
