Healthcare for the Homeless Program

This program provides high-quality, trauma informed, patient-centered care for men, women and children experiencing homelessness and those at risk of homelessness. The program is coordinated with PHMC’s Mary Howard Health Center for the Homeless and the entire PHMC health network, providing primary and behavioral health care and comprehensive social services to meet the complex needs of this vulnerable population.

PHMC runs more than 350 programs providing direct resources and care to thousands of individuals and communities across the region. In addition to running its own programs, PHMC is the parent company to smaller nonprofits, called PHMC Subsidiaries. PHMC and its subsidiaries offer a broad array of programs spanning behavioral health/recovery, nurse-managed primary care, chronic disease management and prevention, tobacco control, obesity prevention, early intervention, HIV/AIDS, violence intervention, homeless health services, parenting supports for families, and much more, plus the research and evaluation that allow us to assess and target health issues effectively.

PHMC continues working for initiatives aimed at improving the quality, efficiency, and overall value of health care. These arrangements provide quality incentives to carry out such improvements and achieve optimal outcomes or our members. For additional tools and resources to support and improve the mental health and wellbeing of all Philadelphians please go to the Healthy Minds Philly website. Healthy Minds Philly is an initiative of the Department of Behavioral Health and Intellectual disAbility Services and serves the people of the City of Philadelphia.
